The “ICU Follow-up Care Lab” provides health monitoring and a range of follow-up services for ICU adult patients following their hospital discharge. Patient follow-up clinics will take place in a hospital setting as scheduled outpatient appointments, post-hospital discharge. Patients will be reviewed/followed up at the hospital where they had been treated.
The main goals of the laboratory are the following:
The timely detection of potentially threatening health conditions in order to maintain and ensure patients’ recovery and good health.
Their immediate and timely referral to the hospital for readmission in order to avoid or treat any adverse outcomes.
The reduction of patient mortality rate outside/post ICU facilities.
To develop strategies that aim to improve patient safety and quality of life
The establishment and strengthening of interdisciplinary collaboration across health care professionals for the best interest of ICU patients.
